Engaging Staff in Individualizing Care: Starter Toolkit

B&F Consulting partnered with Pioneer Network in a National Learning Collaborative funded by The Retirement Research Foundation, in which 49 nursing homes in nine states, affiliated with five culture change coalitions and four corporations, incubated putting in place four foundational practices:

  • Consistent Assignments

  • Huddles

  • Involving CNAs in Care Planning

  • QI Closest to the Resident

and then applied these practices to key clinical areas.

One incubator nursing home’s owner said, “Through this process, we made more progress than we had in the previous fifteen years.” The homes reduced falls, alarms, anti-psychotics, pressure ulcers, and hospitalizations, by tapping into their staff’s knowledge of residents, and individualizing their approach to care accordingly.
